Viêm ph i là gì?
Là b nh lí viêm nhi m nhu mô ph i gây ra b i
viêm đư ng hô h p dư i.
Thư ng x y ra sau 1 đ t nhi m virus đư ng hô h p
trên.V n còn chưa rõ làm th nào mà vi khu n có th
lây lan xu ng đư ng hô h p dư i sau khi g n vào các
receptor disaccharide t bào bi u mô vùng h u h ng.
Sinh lí b nh
Còn nhi u tranh cãi v cơ ch gây b nh:
•
Kháng IgA.
•
Pneumolysins, ch t c ch t ho t đ ng c a các
nhung mao đư ng hô h p.
•
T n thương t bào bi u mô đư ng hô háp do viêm
nhi m trư c đó.
•
Tranh ch p v i các receptor c a các y u t k t t p
ti u c u trên các con đư ng d n t i các ph nang.

Treatment
Remember this as BAPP:
•
Breathing: maintain oxygen saturation levels.
•
Antibiotics: treat the underlying cause (check
hospital guidelines).
•
Pain: give analgesics.
•
Pneumococcal vaccines for those at risk, e.g.
diabetics, the immunosuppressed and those over
65 years old.
Investigations
•
CXR: look for infiltrates.
•
Identify the causative organism by assessing a
sputum sample.
•
Monitor oxygen saturation.
•
Bloods: look for raised WCC and raised
inflammatory markers.
•
Urinary antigen test: for pneumococcal or
Legionella antigen.
•
Arterial blood gas (ABG).
Assess severity using CURB-65
•
Confusion.
•
Urea >7 mmol/L.
•
Respiratory rate >30/min.
•
BP <90/<60 mmHg.
•
>65 years old.
Each section of the CURB-65 is worth 1 point:
1 = Outpatient care.
2 = Admission.
>3 = Requires ICU admission.
Complications
•
Respiratory failure: by causing
acute respiratory distress syndrome (ARDS).
•
Septic shock: the causative agent enters the
patient’s bloodstream, releasing cytokines.
•
Pleural effusion.
•
Empyema.
•
Lung abscess.
•
Hypotension: sepsis or dehydration is usually
the underlying cause.
